**Please print out and complete the new patient forms, along with the medical list form.
Arrive for your appointment 15 minutes before your appointment time.
Please bring your driver’s license, current insurance card, and co-pay (we do accept major credit cards).
Patients 17 and under MUST be accompanied by a parent or legal guardian.
Please bring a list of medications and all tubes of cream (over the counter or prescription) you have been using to treat your rash.
If you are requesting a skin exam, please be prepared to undress and to wear one of our provided gowns. Please DO NOT wear make-up for your exam.
If you cannot make your appointment, please give at least 24 hours' notice for cancellation. ***See "No Show" policy below.
Please review our billing policies before arriving for your appointment.
NO SHOW POLICY: Beginning 1/1/2024, appointments which are not kept will be charged a $50 no-show fee. Thanks for your cooperation.
